a) A backup should be automatically created at least every day that a file is worked on.
b) Command + S should create a snapshot, simply with a timestamp as the title, as this is more convenient.

We create snapshots every time you stop writing for 10 minutes or more, you can find them under Left Sidebar > Drafts > Full History. We also create backups after every writing session (or every 30 minutes when offline). You can create snapshots with Cmd+Shift+S and hitting Enter. Some users are used to hitting Cmd+S every sentence or so out of habit and that would create too many snapshots (or be too obtrusive if we prompted you).
